Abstract
Hospitals as health facilities are required to provide health services to the community as much as possible. One effort to improve the quality of service that is controlling the administration, including administration controlling medical records. Medical Record itself is an activity recording patient information, anamnesis, physical determination of laboratory, diagnostic services and any medical treatment given to the patient and the treatment of both hospitalized and outpatients. Orderly administration will not succeed without the support of the management of medical records is good and right. This research aims to design and build applications in the medical record RSIA Siti Mirza Palembang which can provide information about the patient's medical history. Stages of development of this research method RUP (Rational Unified Process) with Visual Studio 2012 and SQL Server as the database. The system can manage data collection doctor, patient data, installation registration outpatient and inpatient care, record-keeping and reports. There are reports that generate information records of outpatient visits and hospitalizations. System testing technique using black box testing method to check for errors in the system.
